Objective : The main objectives of the study were 1) To implement Kayakalp- Swacchta guidelines in District Hospital Ludhiana and 2) To evaluate the impact on implement the guidelines on cleanliness, hygiene, waste management and infection control practices in the hospital and the hospital boundary

Background : Ministry of Health and Family Welfare launched Kayakalp on May 15, 2015, to promote cleanliness and augment the quality of public healthcare facilities. Kayakalp- Swacchta guidelines have been issued by the Ministry of Health and Family Welfare along with the award scheme to serve as an encouraging factor for the public healthcare facilities. The assessment process of Kayakalp includes four steps: a) Preliminary Internal Assessment, b) Final Internal Assessment, c) Peer Assessment and d) External Assessment. After reviewing these four steps by the external agency, the facilities are ranked based on scores obtained and one's obtaining maximum scores are declared the winners and awarded accordingly. This study is aimed to analyze the impact of the implementation of Kayakalp guidelines on the cleanliness of the hospital. A Pre-intervention study is done to assess the status of cleanliness and analyze the gaps. The identified gaps were fulfilled by special action. The post-intervention study is carried out to understand the impact caused by the implementation of the guidelines in the District Hospital on cleanliness, hygiene, etc.

Methodology : This is an observational study & it is carried at Lord Mahavira Civil Hospital, Ludhiana from February 1, 2018, to April 30, 2018, using the checklist issued by MOHFW for Kayakalp assessment on District Hospitals. The Pre and Post intervention studies were done to evaluate the impact of implementation on Kayakalp guidelines.

Findings : The areas which showed major improvements after the implementation of Kayakalp were Hygiene Promotion (score increased by 58 %), Hospital Upkeep (score increased by 44%), Sanitation and Hygiene (score increased by 38%), Infection Control (score increased by 38%).

Recommendations : Implementation of Kayakalp Swacchta guidelines helped in improving the overall cleanliness in the hospital but due to constraints of finances and manpower, the cleanliness beyond the hospital boundary couldn’t be improved much.
